A care plan is useful if you have an ongoing health condition or several people involved in your care.
A care plan includes what you have planned together with your healthcare team for the next 12 months to improve your health or wellbeing. It means that everyone can be on the same page about what matters to you, such as your goals, values and preferences.
一个emergency or acute plan (also known as an action plan) can be part of your care plan, or may be all that you need. It gives you written information about what to do if you get symptoms or something goes wrong.
- Knowing what to watch out for (your early warning signs) and what you can do at an earlier stage can prevent you getting so sick you need to go to hospital.
- If you don't have one, ask your doctor or nurse for a care plan and/or emergency plan.
